Person Centred Care
We provide person-centred care. We like to get to know the resident, their
personal life history. What they like, what they dislike, what their personal needs are.
We want to know what they did before they came to live with us as well as how they
want to live now.
We see our residents as equal partners in planning, developing and assessing care to
make sure it is most appropriate for their needs. It involves putting residents and their
families at the heart of all decisions. We build a relationship with every resident,
listening to their stories, learning all about their lives, to create a picture of the whole
person. It's about giving residents the choice to live their lives the way they really want
to.
Each resident is allocated a key worker who will be his or her main contact for day-to-
day care delivery. Ensuring continuity of care for the residents' family and friends as
well. A social activities plan and life story are completed together, it is essential that
we know our residents well. If a resident is unable to articulate their likes and dislikes,
their routines, their history, then their family and friends are encouraged to complete
the life story. Wherever possible we want residents to carry on the day-to-day
activities they've enjoyed all their lives. So if you have always enjoyed flower
arranging, gardening or card playing, it's important that these activities continue to be
enjoyed!
